📝 Overview
Admission Agreement - Form ES1100 is the Report Center report that produces the Admission Agreement used when a client enrolls at an adult day care center. ElderSuite pre-fills the agreement with your provider name and the client's name, so it is ready to sign.
The report sits on the Client Reports tab of the Report Center. When you highlight it in the Report List, the Report Description reads "Official Admission Agreement documentation for new client enrollment." Use it when a new client is enrolling, or when an existing client's agreement needs to be printed again.
The Report Center is the only place this report opens. For a tour of the other tabs and reports, see the ElderSuite Report Center Overview.
⚙ How It Works
The Report Opens From Client Reports
In the Report Center, select the Client Reports tab, highlight Admission Agreement - Form ES1100, and click View Report. Double-clicking the entry does the same thing. A small dialog opens over the Report Center, which stays open behind it.
For a Limited Access sign-in, the Report Center requires the Reports Center permission, which an administrator grants on the employee record in the Provider Center. Once the Report Center is open, every report on its tabs is available, including this one.
You Choose the Client; ElderSuite Fills In the Names
The client is the only choice on this dialog. The Select Client selector shows Select a Client until you pick someone, and it lists clients by search name in ascending order. The selector draws on the client information ElderSuite has already loaded for its client list.
ElderSuite fills in two details: the provider name for your current ElderSuite session, and the client name, taken from the selected client's search name. Everything else on the page is the standard agreement content.
The dialog has no date range, no copy count, and no print-format choice. Those printing choices are made when you print the finished agreement.
Preview Produces the Agreement
The dialog has two buttons: Preview and Cancel. There is no separate print button. Clicking Preview shows a please-wait indicator while ElderSuite prepares the agreement, then opens it in the Report Viewer inside the main ElderSuite window. Review it there and print it from the Report Viewer.
The provider name and client name are locked into the finished page, so they cannot be typed over in the Report Viewer. To print a different name, choose the correct client and run the report again.
Producing an agreement does not add, change, or delete anything in ElderSuite. It creates the document only, and each run makes its own copy, so an earlier agreement is not replaced.
Backing Out
Click Cancel, or press Escape, to close the dialog without producing anything. You return to the Report Center.
Messages You May See
If you click Preview before choosing a client, ElderSuite shows Missing Selection ("You must select a client before continuing."). The dialog stays open, so choose a client and click Preview again.
If the agreement template is not installed on the computer, ElderSuite shows Missing Files ("The report you are trying to print is missing. Please re-install ElderSuite."). No agreement is produced. Once ElderSuite has been reinstalled, run the report again from the Report Center.
If the Report Viewer opens without the agreement, it shows File Not Found ("The ElderSuite generated file you are trying to view is not in its expected location. Please try again."). Run the report again from the Report Center.
Other Client Reports work the same way. For example, see How to Print the Contact Information Assessment (Form ES1000) in ElderSuite and How to Print the Attendance Calendar (Form ES3100) in ElderSuite.
⚠ The Dialog Closes After Every Preview
Preview is a one-time action. The dialog closes after it whether the agreement was produced, the template was missing, or an error occurred. To print agreements for several clients, open Admission Agreement - Form ES1100 from the Report Center again for each one.
