How to Print the Contact Information Assessment (Form ES1000) in ElderSuite

Print Form ES1000 from Report Center Client Reports for one client or your active caseload — contacts, emergency people, physician, pharmacy, and more.

📝 Overview

Contact Information Assessment - Form ES1000 is the Report Center report that prints one filled contact and emergency information sheet per selected client. Each page gathers contact details, emergency contacts, care contacts, enrollment information, advance directives, visible custom fields, and the client’s photo when one is available.

Open Report Center from the ElderSuite main menu, select the Client Reports tab, and choose Contact Information Assessment - Form ES1000. The Report Description reads “Complete Contact Information Assessment for a client which includes contacting information, emergency contacts, physicans, pharmacies, and more.”

The same Form ES1000 document can also be produced from Client Information: open the client, then click Print on the toolbar. That path prints only the client on screen. Use the Report Center when you need several clients — or the whole active caseload — in one continuous document.

This report is not Client Roster - Form ES1200 and not the CSV from Export Client Records. The roster is a compact list with optional extra columns; Form ES1000 is the full contact and care sheet. ⚙ How It Works

1. Open the Report

From Report Center → Client Reports, highlight Contact Information Assessment - Form ES1000 in the Report List. Click View Report, or double-click the report name. The options dialog opens over the Report Center with nothing checked.

For a Limited Access sign-in, Report Center requires the Reports Center permission. Without it, the menu item is unavailable. Permissions are read at sign-in, so sign out and back in after the permission is granted. A Full Access sign-in can open the Report Center.

From Client Information, click Print on the toolbar for the open client. If there are unsaved changes, ElderSuite asks whether to save before printing. That single-client route does not open the Report Center selection window.

2. Select One or More Clients

In the Report Center options window, check each client who should get a sheet, or check Select All Clients above the list.

Only clients that are not marked inactive appear in the list. Checkboxes are operated with the mouse; the list does not take keyboard focus for checking.

When Select All Clients is checked, the list is grayed out so you cannot trim the selection while it stays checked. Clearing Select All Clients unchecks everyone rather than restoring a prior selection, so you start again from nothing.

At least one client must be checked before Preview will run. If none are checked, ElderSuite shows No Client Selected and the window stays open.

3. Preview the Assessment

Click Preview. A please-wait indicator appears while ElderSuite builds a Form ES1000 page for each checked client in list order, joins the pages into one continuous document, and opens it in Report Viewer inside the main ElderSuite window. The options dialog then closes.

Preview does not send the document straight to a printer. Print or save from Report Viewer.

Click Cancel (or the title-bar close control) to return without producing a report. This window only reads client information; it does not add, change, or delete any client record.

4. Read What the Sheet Shows

Each selected client gets one filled page in the same continuous document. Pages follow the order clients appear in the list; this window offers no way to reorder them.

Each page can include identification details (including date of birth and the age worked out from it), contact details with formatted phones and address blocks, alerts, enrollment details (including payment type, schedule, and authorization dates), primary and secondary contacts, case worker, physician, pharmacy, hospital, advance directives (Living Will, DNR, Durable Power of Attorney, and Medical Power of Attorney when set on the record), up to ten visible custom fields with the provider’s labels, and the client’s photo in the reserved space when a photo is available.

Whatever is saved on the client record is what prints. Empty fields print blank. Values on the finished page cannot be typed over in Report Viewer. To change what prints, update the client record and run the report again.

✅ Important Limitations

Only clients that are not marked inactive appear in the Report Center list. A client marked inactive cannot be chosen from this window. For that client’s Form ES1000, open the record in Client Information and use Print.

A client whose page cannot be built is left out and the run continues with the next client, so a combined document can contain fewer clients than were checked.

If not a single page can be built, the options window closes and no Report Viewer opens. There is no separate “report failed” message on this window; read any message that appeared while the pages were being attempted.

The report layout is fixed. You choose which clients to include; you do not pick columns the way Client Roster - Form ES1200 does with Customize. Custom fields print only when marked visible in the provider’s user-defined field setup.

This window only reads client information. It does not add, change, or delete client records.

This is not Client Roster - Form ES1200 and not the CSV from Export Client Records. Form ES1000 prints the full contact and care sheet.

If the Form ES1000 template is missing on that computer, ElderSuite shows Missing Files (“The report you are trying to print is missing. Please re-install ElderSuite.”).

⚠ No Client Selected

From the Report Center options window, Preview needs at least one checked client. With none checked, ElderSuite shows No Client Selected (“You must select at least one client before continuing.”), no document is built, and the window stays open so you can check clients and try again.
