📝 Overview
The Health Assessment & Service Plan is the ElderSuite record staff use to keep a client's health assessment and individual service plan. Open it from the Nursing Center. ElderSuite prints Health Assessment & Individual Service Plan - Form 3050 from that saved record when a paper copy is needed.
ElderSuite serves adult day care centers nationwide. The printed form is Form 3050.
Staff record the applicable information on the assessment. This is a dedicated Health Assessment workflow. It is not a generic document-storage screen, and staff do not fill a downloaded 3050 PDF by hand.
The Health Assessment sits with the client's other Nursing Center records, including Physician's Orders and Monthly Nursing Assessments. Those are separate records. Form 3050 and Form 3055 are not the same record.
Governing program requirements decide when Form 3050 is required. ElderSuite does not.

âš™ How It Works
1. Open Health Assessment & Service Plans
Open the Nursing Center and click Health Assessment & Service Plans. Health Assessment List opens with clients on the left and the selected client's records on the Assessments tab.
Highlight the client you want. Each Health Assessment belongs to one client. Work from that client's record.
For a Limited Access sign-in, Health Assessment & Service Plans can be unavailable until the matching Nursing Center permission is granted on the employee record.
The same list can also open from Report Center by selecting Health Assessment & Individual Service Plan - Form 3050. Form 3050 still prints from the open Health Assessment record, not from the list heading.
2. Create and Maintain the Record
Click Add New to start a Health Assessment for the selected client, or open an existing record with Edit or a double-click.
Complete the applicable information. Keep a complete Assessment Date on the record. A selected client in Select Client and a complete Assessment Date are required before ElderSuite will save the record.
Click Save & Close to store the Health Assessment. After the first successful save, that record stays with the selected client. Select Client is then turned off, so a record filed against the wrong client has to be deleted and started again.
3. Print Form 3050 From That Record
Open the saved Health Assessment and click Print. ElderSuite fills Health Assessment & Individual Service Plan - Form 3050 and opens it in the Report Viewer.
Printing produces the completed form from the information already maintained on the client's Health Assessment. It does not download a blank government PDF for staff to fill by hand.
Print also requires a client and a complete Assessment Date. If the record has unsaved changes, ElderSuite asks whether to save first.
4. File the Signed Copy
ElderSuite maintains the Health Assessment and prints Form 3050. The printed form is not signed. The signed copy that returns is kept as a client document.
From the saved record, Sign & Sync opens the ElderSuite Scan Manager so the signed paper copy can be scanned into the client's documents. View Signed Copy appears on the menu bar once a signed copy is on file.

5. Set the Assessment to Recur
On the open Health Assessment, click Make Recurring. Health Assessment - Setup Recurring Record opens so the assessment can repeat on a schedule. When the record is already recurring, the same command reads Edit Recurring Schedule.
A recurring record is a template the schedule creates assessments from. Open the template from the Recurring Assessments tab. Work the assessments the schedule created from the Assessments tab.
Sign & Sync is disabled on the recurring template. Open an assessment the schedule created to print Form 3050 or file a signed copy.
âš Save Needs a Client and Assessment Date
Save & Close stores the Health Assessment only when a client is selected in Select Client and Assessment Date is filled in. If either is missing, the window stays open and nothing is saved.
Check Select Client and Assessment Date, then click Save & Close again.
