Physician's Order - Form 3055 in ElderSuite

Maintain a client's Physician's Order in the Nursing Center, print Physician's Order - Form 3055 from the saved record, and file the signed copy with the client.

📝 Overview

ElderSuite maintains each client's Physician's Order as a dedicated clinical record in the Nursing Center. Staff create or update the order in ElderSuite and print the completed Physician's Order - Form 3055 from that saved record when a paper or signed copy is needed.

This is not a generic document-storage workflow, and staff do not need to maintain a separate blank PDF outside ElderSuite. The Physician's Order record is the working source for the information that appears on the printed form.

Requirements for physician orders vary by state, payer, program, and center policy. ElderSuite provides the recordkeeping and printing workflow; it does not determine when a new Physician's Order is required.

âš™ How It Works

1. Open Physician's Orders

Open the Nursing Center and click Physician's Orders. The Physician's Order List opens with the client's orders.

Select the client you want to work with. Each Physician's Order belongs to one client, so the order is maintained as part of that client's clinical record rather than from a shared blank form.

For a Limited Access sign-in, Physician's Orders requires the Edit Physician's Order - Form 3055 permission.

2. Create or Maintain the Physician's Order

Click Add New to start a Physician's Order for the selected client, or open an existing order with Edit or a double-click.

Complete the applicable order information. Choose the ordering physician in Select Physician and keep a complete Date on the record. A selected client and a complete Date are required before the order can be saved or printed.

Click Save & Close to store the Physician's Order with the selected client.

Diagnoses, medications, and treatments used on the order are the client's clinical records. They can be added or edited from the Physician's Order, and they are also maintained through Diagnoses, Medications, and Treatments in the Nursing Center.

The Physician's Order sits alongside other Nursing Center records such as Health Assessment & Service Plans, Monthly Nursing Assessments, and Nurse's Notes. These are separate records; ElderSuite does not treat Form 3055 and Form 3050 as the same record.

3. Print Form 3055 From the Saved Record

Open the saved Physician's Order and click Print. ElderSuite produces the completed Physician's Order - Form 3055 from the information already maintained on the client's order and opens it in the Report Viewer.

Staff do not need to locate an old blank Form 3055 PDF on the ElderSuite website and maintain a second copy of the same information.

If no client is selected or Date does not contain a complete date, the record is not ready to print.

4. Obtain and File the Signed Copy

ElderSuite maintains the Physician's Order and prints Form 3055. It does not fax, email, or portal-send the order to the physician, and it does not obtain the physician's signature electronically through this workflow.

After the center follows its normal process for obtaining the physician's signature, the signed copy can be kept with the chart or stored with the client's ElderSuite documents.

From a saved Physician's Order, Sign & Sync can open the ElderSuite Scan Manager so the signed paper copy can be scanned into the client's documents. Staff can also scan or attach the signed copy through Client Documents.

5. Create a New Order When One Is Needed

ElderSuite does not give Physician's Orders a recurring schedule and does not create the next Form 3055 automatically. There is no Make Recurring command on this record.

A Health Assessment and a Monthly Nursing Assessment can each be set to recur. A Physician's Order cannot. When the applicable state, payer, program, physician, or center policy requires a new order, create a new Physician's Order for that client.

âš  Printing Is Not a Signature

Print produces the completed Physician's Order - Form 3055 for the center's signature workflow. Printing the form does not mean it has been signed or approved.

Obtain the physician's signature using the center's applicable process, then keep the signed copy with the chart or store it with the client's documents in ElderSuite.
