How to Complete a Social History Assessment (Form ES2000) in ElderSuite

Record a client's social background on a Social History Assessment, print it on Form ES2000 for signature, scan the signed copy back with Sign & Sync, and keep it with the client's documents.

📝 Overview

A Social History Assessment records a client's social background for care planning at your adult day care center: religion, marital status, occupation, living arrangements, physical and mental history, languages, hobbies, involvement with organizations, and free-form comments. ElderSuite prints it on Form ES2000 - Social History Assessment so the client or responsible party can sign it, and the signed copy is kept with the client's documents.

Centers typically record one at admission and add a fresh one at each reassessment. All of a client's assessments, and their signed copies, live on the Social History Assessment List.

⚙ How It Works

The Social History Assessment List

Open the Client Center and click Social History Assessments. The same list also opens from the Report Center: on the Client Reports tab, select Social History Assessment - Form ES2000 and click View Report, or double-click it. For a Limited Access sign-in, the Client Center button is available only when an administrator grants the matching permission on the employee record in the Provider Center; see the ElderSuite Client Center Overview.

The list shows your clients on the left. For the client you select, the right side has a Social History Assessments tab, sorted by Assessment Date with the newest first, and a Sign & Sync Documents tab with the signed copies. The View dropdown starts on Active Clients; switch it to All Clients to include inactive clients.

Select the client, then click Add New (Alt+N) to start an assessment, or double-click an assessment, or select it and click Edit (Alt+E), to open one on file. The new assessment is attached to whichever client is selected. Only one assessment window is open at a time, so finish and save the one you are working on before opening another.

Filling In the Assessment

A new assessment opens with the client already filled in, today's date in the Date box, and Print Later checked. The window scrolls through the General History, Physical & Mental History, Extracurricular Activities, and Comments groups.

Most answers are chosen from dropdown lists. The short typing boxes, such as languages spoken, hobbies, and involvement with organizations, have a dropdown arrow that opens a larger panel for longer answers, and they stop accepting text at 255 characters. Comments has no length limit, so longer detail belongs there.

No answer is required. An assessment saves with only the client and a complete date. Once an assessment is saved, its client cannot be changed.

Saving

Save & Close saves the assessment and returns you to the list, where it now shows. If you close the window with unsaved changes, ElderSuite asks "Would you like to save your changes?": Yes saves and closes, No closes without saving, and Cancel keeps the window open.

Saving also keeps the Social History item on the client's intake check list up to date. It is turned on when the client has at least one Social History Assessment and off when the last one is deleted. For the client record and its intake check list, see How to Add a Client in ElderSuite.

Printing Form ES2000

Click Print on the assessment window. If there are unsaved changes, ElderSuite asks "You must save your changes before printing. Would you like to save your changes and print?" Choose Yes to save and print. The filled form opens in the Report Viewer, where you print it or save a copy; see How to Use the Report Viewer in ElderSuite.

The printed form carries the assessment answers, the comments, and the client's name in the footer. The assessment date and the Print Later mark do not print. The filled form is locked in the Report Viewer; to change it, edit the assessment and print again. Printing unchecks Print Later, so ElderSuite may ask to save when you close the window afterward.

If the blank form is not installed on the computer, ElderSuite shows "The report you are trying to print is missing. Please re-install ElderSuite." and nothing prints. Reinstall ElderSuite on that computer, then print again.

Scanning the Signed Copy With Sign & Sync

After the printed page is signed, open the assessment again and click Sign & Sync. ElderSuite Scan Manager opens so you can scan the signed page. When the scan is complete, ElderSuite shows "The signed copy has been scanned. It will be added to the client's documents when you save." The Signed & Synced badge and the View Signed Copy button appear on the assessment.

Each assessment holds one signed copy. If one is already on file, ElderSuite asks before replacing it. If no client is selected or the date is incomplete, ElderSuite shows "Please select a client and enter the assessment date before signing and syncing." For more on scanning signed paperwork, see How to Use Sign & Sync in ElderSuite.

Once saved, the signed copy appears in the client's documents and on the list's Sign & Sync Documents tab. Double-click it there, or click View Signed Copy on the assessment, to open it.

Deleting an Assessment

Open the assessment and click Delete. ElderSuite asks you to confirm, and No is the selected answer, so pressing Enter cancels. When the assessment has a signed copy, the confirmation warns: "This record has a signed copy on file. Deleting the record will also remove the signed copy from the client's documents." Answering Yes deletes the assessment together with its signed copy.

When Someone Else Changes the Record

If another user changes or removes the same assessment while you have it open, ElderSuite shows "This record has changed since you last opened it and your changes were not saved. Please open the record again to make changes." Your changes are not saved and the window closes. Open the assessment again from the list and make your changes once more.

⚠ Save After You Scan

A scanned signed copy is not added to the client's documents until you save the assessment. If you close the window and answer No to saving, the scan is discarded. After Sign & Sync, click Save & Close.
