How to Print the Daily Meal Count and Attendance Record (Form 1535) in ElderSuite

Print the weekly Form 1535 from Report Center Nutrition Reports — blank for hand entry at the start of the week, or completed with attendance and meal marks for review.

📝 Overview

Daily Meal Count & Attendance - Form 1535 is the Report Center report that prints a Monday-through-Friday meal count and attendance grid for nutrition program records. ElderSuite builds one week at a time: client names and ages, the five weekday dates, and — when you choose — attendance and meal marks with daily totals.

Open Report Center from the ElderSuite main menu, select the Nutrition Reports tab, and choose Daily Meal Count & Attendance - Form 1535. The window that opens is titled Daily Meal Count & Attendance Record - Form 1535. The form number is the reliable match.

This report opens only from Report Center → Nutrition Reports. It is not in the Nutrition Center. It is also not the same as the Client Reports attendance prints (for example Form 3683). Form 1535 is the weekly nutrition meal-count sheet — the same layout Scan Meals is built to read when staff mark a blank log by hand.

⚙ How It Works

1. Open the Report From Report Center

Open Report Center and select Nutrition Reports. In the Report List, highlight Daily Meal Count & Attendance - Form 1535. The Report Description reads "Daily record of meals served and client attendance for nutrition program compliance."

Click View Report, or double-click the report name. The options dialog opens centered on the Report Center and keeps the Report Center busy until the dialog closes.

No client needs to be selected first. The week includes every client enrolled at any point during those five days.

2. Confirm the Monday-to-Friday Week

The dialog opens with Start Date set to the Monday of the week that contains today and End Date set to that week's Friday. For the current week you can leave the dates as they are.

Form 1535 covers one Monday-through-Friday week only. Setting a Monday in Start Date fills End Date with that week's Friday. Setting a Friday in End Date fills Start Date with that week's Monday.

If Preview runs while the dates are not a matching Monday and Friday for the same week, ElderSuite shows Invalid Dates with the message that this is a weekly report and you must choose five consecutive days starting on a Monday and ending on a Friday. Correct the dates and try again. For several weeks, run the report once per week.

3. Choose a Completed Record or a Blank Hand Log

Show Client Meals Served & Total Meals Served sits below the date fields and is checked by default. Click it to change the state (it does not take keyboard focus when tabbing).

  • Leave it checked for a completed week: the grid prints an "X" for each day attended and each meal served (Breakfast, Morning Snack, Lunch, Afternoon Snack), plus daily totals at the foot of each page.
  • Clear it for a blank recording sheet: client names, ages, and the five weekday dates print, but meal and attendance marks and totals stay empty for staff to complete by hand — the usual setup before Scan Meals.

4. Preview and Print

Click Preview. ElderSuite builds the week as a single document in the report viewer inside the main ElderSuite window. Print or save from the viewer. The options dialog then closes.

Click Cancel (or press Esc) to close without producing a report.

Page headings draw the contractor name and contract entity identifier from the food contractor record and the site name from the provider settings for the current session. Those values are not typed on this dialog.

✅ Important Limitations

One Monday-to-Friday week per run. Weekend attendance that may exist elsewhere in ElderSuite is outside this Form 1535 printout.

The report reads enrolled clients and saved attendance and meal information for that week. It does not add, change, or delete Meal Records or attendance.

A week with no clients enrolled produces no pages. ElderSuite may show that the report was not generated successfully even when nothing is "broken" — confirm enrollment for that week, then reopen the report from Report Center.

If the blank Form 1535 file is missing from the ElderSuite forms folder on this computer, Missing Files appears and the run fails until ElderSuite is reinstalled and the form files are restored.

⚠ Unchecked Means a Blank Grid on Purpose

When Show Client Meals Served & Total Meals Served is cleared, the printout has no "X" marks and no totals by design. That is the hand-entry sheet. Leave the box checked when you need the completed week with attendance and meal marks filled from ElderSuite.
