How to Record Vital Signs in ElderSuite

📝 Overview

ElderSuite does not have a separate vitals screen. Blood pressure, pulse, respiration, temperature, height, weight, and blood sugar are recorded on Page 2 of a nursing assessment—either a Health Assessment & Service Plan (Form 3050) or a Monthly Nursing Assessment (Form ES3500).

ElderSuite serves adult day care centers nationwide.

The Nursing Center has eight activity buttons. None of them is Vitals. Open the assessment that matches the documentation you are completing, go to Page 2, and use section II J. Vital Signs/Height/Weight/Blood Sugar.

ElderSuite stores and prints the readings you enter. It does not decide which vitals are required or how often they must be taken—that remains a clinical, state, payer, and program decision for the center.

⚙ How It Works

1. Choose the Right Assessment

Use a Health Assessment & Service Plan when you are maintaining Form 3050 for the client.

Use a Monthly Nursing Assessment when you are completing the four-page monthly assessment that prints as Form ES3500.

Both assessments use the same Page 2 vitals section with the same seven boxes. A Nurse's Note is a different record and is not where vitals are entered.

2. Open a Health Assessment (Form 3050)

Open Nursing Center → Health Assessment & Service Plans. The Health Assessment List opens with clients on the left.

Highlight the client. Stay on the Assessments tab (the tabs are Assessments, Recurring Assessments, and Sign & Sync Documents).

Click Add New for a new assessment, or highlight a row and click Edit (or double-click it).

Go to Page 2, section II J. Vital Signs/Height/Weight/Blood Sugar. Enter the readings. Additional Comments sits directly beneath the vitals boxes and prints with Section II J on Form 3050.

3. Open a Monthly Nursing Assessment (Form ES3500)

Open Nursing Center → Monthly Nursing Assessments. Select the client on the left.

Click Add New, or open an existing assessment with Edit or a double-click.

The assessment spans four pages. Vitals are still section II J on Page 2. Enter the readings there, then continue the rest of the assessment as needed.

The completed assessment prints as Form ES3500. In the Report Center, the same list can appear under Nursing Assessment - Form ES3050—that entry opens the Monthly Nursing Assessment list; the printed form is still ES3500.

4. Enter the Seven Readings

In J. Vital Signs/Height/Weight/Blood Sugar, type into:

• Blood Pressure
• Pulse
• Respiration
• Temp. (optional)
• Height
• Weight
• Blood Sugar (optional)

Each box accepts free text up to 15 characters. ElderSuite does not validate the format and does not calculate anything from what you type. Two captions say optional; in practice none of the seven is required to save.

5. Save & Close

Click Save & Close to store the assessment with the selected client.

A selected client and a complete Assessment Date are required before the assessment will save. The vitals boxes themselves can all be blank and the assessment can still save and print.

To put the readings on paper, open the saved assessment and use Print so Form 3050 or Form ES3500 opens in the Report Viewer with Section II J filled from what you entered.

✅ Important Limitations

There is no Vitals button, Vitals list, or standalone vitals module in the Nursing Center.

Vital signs are not entered on a Nurse's Note, a Physician's Order, or a medication record.

ElderSuite does not enforce a required set of vitals or a recording schedule. Blank vitals boxes do not block save or print when the client and assessment date are complete.

Each vitals box is limited to 15 characters. Longer readings will not fit.

⚠ There Is No Separate Vitals Screen

If you are looking for a Vitals command in the Nursing Center, you will not find one. Open the Health Assessment or Monthly Nursing Assessment for the client and enter the readings on Page 2, section II J.
