How to Complete Monthly Nursing Assessments in ElderSuite

📝 Overview

The Monthly Nursing Assessment is a four-page client record in the Nursing Center. The completed assessment prints as Form ES3500.

This is separate from a Nurse's Note and from the Health Assessment & Individual Service Plan - Form 3050.

In some ElderSuite labels, including the permission and Report Center entry, the Monthly Nursing Assessment is identified as Nursing Assessment - Form ES3050. Those labels open the Monthly Nursing Assessment workflow; the completed assessment itself prints as ES3500.

⚙ How It Works

1. Open Monthly Nursing Assessments

Open Nursing Center → Monthly Nursing Assessments.

Select the client on the left. The Assessments tab contains saved monthly assessments, while Recurring Assessments contains recurring templates and schedules.

For a Limited Access user, the Nursing Center route requires the nursing-assessment editing permission. Permission changes take effect after the employee signs in again.

The same list can also be opened from Report Center → Nursing Reports → Nursing Assessment - Form ES3050.

2. Complete the Four-Page Assessment

Click Add New for the selected client, or open an existing assessment for editing.

All four pages belong to one record and are saved together. Complete the assessment information needed for the current period, including a complete Assessment Date.

The Completed by selection supplies the nurse name used on the printed form.

The medications grid reflects the client's medication records. Medications are viewed on the assessment but added or edited through the separate Medication window opened from the assessment.

3. Save and Print Form ES3500

Click Save & Close to store the assessment.

Use Print to open the completed Monthly Nursing Assessment - Form ES3500 in the Report Viewer. Save any pending changes before printing.

From the assessment list, Selected Record prints one assessment, 'Print Later' Records prints records marked for later printing, and Print by Date Range produces a combined document containing every existing Monthly Nursing Assessment dated within the selected range.

Print by Date Range accepts a range of up to 31 days and does not require a client or assessment to be selected first.

4. Set a Recurring Schedule When Needed

Open the assessment and click Make Recurring. Choose Monthly or Yearly and set a future Next Due Date.

When a recurring assessment becomes due, ElderSuite asks at sign-in for permission to create the due recurring record. If the user approves, ElderSuite creates the record, calculates the next due date, and reschedules the recurring template.

The generated assessment is pre-filled from the recurring record and remains editable. Review and update it so it reflects the client's current condition and services before printing or filing it.

When recurrence is first created, Copy preserves the existing assessment and creates a separate recurring template. Convert turns the current assessment into the recurring template.

✅ Important Limitations

A client and complete Assessment Date are required for the assessment to save or print correctly.

A saved assessment remains assigned to its client.

Printing is performed from ordinary assessment records rather than from the recurring-template tab.

Recurring schedules create pre-filled future assessments; staff still need to review and update the new record for the current period.
