How to Print the Treatment Administration Record (Form ES4020) in ElderSuite
Print the weekly Treatment Administration Record (Form ES4020) from Report Center Nursing Reports—or from Treatments—for a Sunday-to-Saturday week for one or more clients.

📝 Overview
Treatment Administration Record - Form ES4020 is the Report Center report that produces the weekly paper treatment sheet for tracking and compliance. Each page lists a client's treatments for one Sunday-to-Saturday week, with seven dated day columns left blank for staff to mark by hand.
ElderSuite serves adult day care centers nationwide.
Open Report Center from the ElderSuite main menu, select the Nursing Reports tab, and choose Treatment Administration Record - Form ES4020. The Report Description reads "Weekly record of all treatments administered to clients for tracking and compliance."
The same options dialog also opens from Nursing Center: click Treatments, highlight a client, then choose Print → Print Treatment Administration Record. That path starts with the highlighted client already checked.
Form ES4020 is the blank weekly paper sheet. The monthly TAR printed from eTAR is a different report on the same Nursing Reports tab—see How to Use eTAR in ElderSuite. ⚙ How It Works
1. Open Form ES4020
From Report Center → Nursing Reports, highlight Treatment Administration Record - Form ES4020 in the Report List. Click View Report, or double-click the report name. The dialog opens over the Report Center with nothing checked in Select Client(s), and the week already set to the current Sunday through Saturday.
From the Nursing Center, open Treatments, highlight the client on the left, click Print, then Print Treatment Administration Record. The same dialog opens with that client already checked so Preview can run after you confirm the week.
2. Confirm the Sunday-to-Saturday Week
Form ES4020 covers exactly one week: Start Date must be a Sunday and End Date the Saturday six days later.
The dialog opens on the current week. Enter a Sunday in Start Date and End Date fills in that week's Saturday. Enter a Saturday in End Date and Start Date fills in that week's Sunday. You rarely need to set both fields by hand.
If Preview runs with any other range, ElderSuite shows Invalid Dates with the message that this is a weekly report and you must choose seven consecutive days starting on a Sunday and ending on a Saturday.
3. Select One or More Clients
Open Select Client(s) and check each client who should get a weekly sheet. More than one client can be checked in the same run.
Only active clients appear in the list. At least one client must be checked before Preview will run. If none are checked, ElderSuite shows No Client Selected ("You must select at least one client before continuing.") and nothing else happens.
4. Preview and Print From Report Viewer
Click Preview. ElderSuite builds a filled ES4020 page for each page of treatments for every checked client, joins those pages into one document in client name order, opens the document in Report Viewer inside the main ElderSuite window, and closes the dialog.
Print or save from the viewer. Click Cancel to close without producing a report. This window only reads treatment records; it does not add, change, or delete them.
5. Read What the Sheet Shows
Each page is a filled blank ES4020 for one client and one week. The heading carries the client's name, the reporting period as the start date "to" the end date, and the seven dated day columns Sunday through Saturday.
Up to six treatment rows print per page. Each row shows the treatment, its frequency, its notes, and its administration times. Times that are not set on the treatment record print blank. A client with more than six treatments gets continuation pages automatically.
The day columns themselves are blank. This is the paper sheet staff initial by hand. The filled-in monthly record comes from eTAR, not from this print.
Only treatments that are not marked Inactive and have Include on Treatment Sheet checked appear on the sheet. That same switch is what puts a treatment on eTAR.
✅ Important Limitations
Form ES4020 accepts only one Sunday-to-Saturday week. Any other date range is refused when you click Preview.
Only active clients appear in Select Client(s). A client marked inactive cannot be chosen from this window.
A treatment prints here only when it is not Inactive and Include on Treatment Sheet is checked. The day-of-week boxes, Start date, and Stop date on the Treatment window do not filter this paper sheet—those settings belong to eTAR.
The day columns on the printed page are blank for hand marking. This print does not record who administered a treatment or when.
A checked client with no qualifying treatments contributes no pages. If every checked client has none, ElderSuite shows No Records Found, the dialog stays open, and the client list resets to active clients only.
This window changes no client or treatment record. It is not the monthly Treatment Administration Record - eTAR (Monthly) report and not the medication Form ES4010.
If the ES4020 template is missing on that computer, ElderSuite shows Missing Files ("The report you are trying to print is missing. Please re-install ElderSuite.").
⚠ Use One Sunday-to-Saturday Week
Form ES4020 is a weekly sheet. Start Date must be a Sunday and End Date that week's Saturday.
Set either date to the correct Sunday or Saturday and let the other fill in. If Preview reports invalid dates, fix the week before trying again—ElderSuite will not produce a partial week or a multi-week range from this dialog.
