How to Print the Diagnosis Profile (Form ES4030) in ElderSuite

📝 Overview

Diagnosis Profile - Form ES4030 is the Report Center report that prints every diagnosis on file for selected clients. Each page lists the ICD code, the diagnosis, the onset date, whether it is Active or Inactive, and the notes recorded on that diagnosis.

Open Report Center from the ElderSuite main menu, select the Nursing Reports tab, and choose Diagnosis Profile - Form ES4030. The Report Description reads "Every diagnosis on file for a client - ICD code, onset date, status and notes - active diagnoses first. The Diagnoses List counterpart of the Medication Profile."

The same options window also opens from the Nursing Center: open Diagnoses, highlight a client, then choose Print and Print Diagnosis Profile. That path starts with the highlighted client already checked.

This report is the Diagnoses List counterpart of Medication Profile - Form ES4000. It is not a Client Reports attendance print (for example Form ES3100), not the Client Roster, and not the weekly nutrition Form 1535. ⚙ How It Works

1. Open the Report

From Report Center -> Nursing Reports, highlight Diagnosis Profile - Form ES4030 in the Report List. Click View Report, or double-click the report name. The options dialog opens over the Report Center with nothing checked in Select Client(s).

From the Nursing Center, open Diagnoses, highlight the client on the left, click Print, then Print Diagnosis Profile. The same dialog opens with that client already checked so Preview can run right away.

2. Select One or More Clients

Open Select Client(s) and check each client who should get a profile. More than one client can be checked in the same run.

Only active clients appear in the list. Type while the dropdown has focus to jump to a matching name.

At least one client must be checked before Preview will run. If none are checked, ElderSuite shows No Client Selected and nothing else happens.

3. Preview the Profile

Click Preview. ElderSuite reads the diagnoses again from what is saved now, sorts the checked clients by search name, and builds one document for every selected client who has diagnoses on file.

The document opens in Report Viewer inside the main ElderSuite window. The options dialog then closes. Print or save from the viewer.

Click Cancel (or the title-bar close control) to return without producing a report. This window only reads diagnoses; it does not add, change, or delete them.

4. Read What the Profile Shows

Each selected client with diagnoses on file gets one or more portrait pages in the same style as the Client Roster. Pages for several clients are joined into one document in search-name order.

Each page header shows the client's search name, the date the profile was printed, and Page N of M. Below the header, up to fourteen diagnosis rows print per page: ICD Code, Diagnosis, Onset Date, Status (Active or Inactive), and the notes under each line.

Active diagnoses print first, then inactive ones, each group in alphabetical order. Inactive diagnoses are included; there is no option on this window to leave them out. Values on the page cannot be edited in Report Viewer. To change what prints, correct the diagnosis on the Diagnoses list and run the report again.

✅ Important Limitations

Only active clients appear in Select Client(s). A client marked inactive cannot be chosen from this window.

A checked client with no diagnoses on file gets no empty page. If every checked client has no diagnoses, ElderSuite shows No Records Found, no profile opens, and the client list resets to the full active list.

The report reads saved diagnoses. It does not add, change, or delete diagnosis records.

This is not Medication Profile - Form ES4000, not Form ES3100, not the Client Roster, and not Form 1535. Form ES4030 prints diagnoses only.

If the ES4030 report template is missing on that computer, ElderSuite shows Missing Files ("The report you are trying to print is missing. Please re-install ElderSuite.").

⚠ No Client Selected

From the Diagnoses list, Print Diagnosis Profile needs a highlighted client. With none highlighted, ElderSuite shows No Client Selected ("Please select a client first.") and the options window does not open.

From the options window itself, Preview needs at least one checked client. With none checked, the same No Client Selected message appears and no profile is built.
