How to Print the Medication Profile (Form ES4000) in ElderSuite

Print the Medication Profile (Form ES4000) from Report Center Nursing Reports—or from Medications—for one or more clients with prescriptions, dosages, and schedules.

📝 Overview

Medication Profile - Form ES4000 is the Report Center report that produces a complete printable medication record for one or more clients. Each page shows two medications with prescription details, dosage and schedule information, the related diagnosis, and the prescribing physician's and procuring pharmacy's contact details.

ElderSuite serves adult day care centers nationwide.

Open Report Center from the ElderSuite main menu, select the Nursing Reports tab, and choose Medication Profile - Form ES4000. The Report Description reads "Complete Medication Profile listing all current prescriptions, dosages, and schedules."

The same options dialog also opens from Nursing Center: click Medications, highlight a client, then choose Print → Print Medication Profile. That path starts with the highlighted client already checked.

Form ES4000 is the full medication profile. The weekly signing sheet is Medication Administration Record - Form ES4010, and the monthly filled record comes from eMAR—see How to Add Client Medications and Print the MAR and How to Use eMAR in ElderSuite. Medications themselves are maintained under Nursing Center Medications. This Guide covers the Medication Profile print only.

⚙ How It Works

1. Open Form ES4000

From Report Center → Nursing Reports, highlight Medication Profile - Form ES4000 in the Report List. Click View Report, or double-click the report name. The dialog opens over the Report Center with nothing checked in Select Client(s).

From the Nursing Center, open Medications, highlight the client on the left, click Print, then Print Medication Profile. The same dialog opens with that client already checked so Preview can run after you confirm the selection. If no client is highlighted, ElderSuite shows No Client Selected ("Please select a client first.") and the dialog does not open.

2. Select One or More Clients

Open Select Client(s) and check each client who should get a profile. More than one client can be checked in the same run.

Only active clients appear in the list. At least one client must be checked before Preview will run. If none are checked, ElderSuite shows You must select at least one client before continuing. and nothing else happens.

There is no date range on this dialog. The only choice is which clients to include.

3. Preview and Print From Report Viewer

Click Preview. ElderSuite shows a Working... message while it reloads medications, diagnoses, physicians, and pharmacies, fills a blank ES4000 page for each pair of medications for every checked client, merges those pages into one document in client name order, opens the document in Report Viewer inside the main ElderSuite window, and closes the dialog.

Print or save from the viewer. Click Cancel to close without producing a report. This window only reads medication records; it does not add, change, or delete them.

4. Read What the Profile Shows

Each page is a filled ES4000 for one client. The heading carries the client's name and Medicaid number. Two medication slots print per page. A client with an odd number of medications gets a blank second slot on the last page. Continuation pages are added automatically.

Each medication slot shows whether the medication is included on the administration record, the medication name, related diagnosis, issue date, start date, stop date, prescription number, amount issued, frequency, strength, dosage, route, directions, administration method, and up to five administration times. Blank dates and times print blank.

The same slot also prints the prescribing physician's name, phone, fax, email, license number, license type, and addresses, and the procuring pharmacy's name, phone, fax, email, and addresses.

The finished report is locked so its entries cannot be typed over in the viewer.

✅ Important Limitations

This dialog has no date range, sorting, or medication filter. The only selection is which active clients to include.

Only active clients appear in Select Client(s). A client marked inactive cannot be chosen from this window.

A checked client with no medications recorded contributes no pages. If every checked client has none, ElderSuite shows No Records Found with the message that no Medication Administration Records were found for the client selection, the dialog stays open, and the client list resets to active clients only.

This window changes no client or medication record. It is not the weekly Medication Administration Record - Form ES4010 and not the monthly Medication Administration Record - eMAR (Monthly) report.

If the ES4000 template is missing on that computer, ElderSuite shows Missing Files ("The report you are trying to print is missing. Please re-install ElderSuite.").

⚠ Choose the Medication Profile Not the MAR

Medication Profile - Form ES4000 and Medication Administration Record - Form ES4010 sit next to each other on the Nursing Reports tab and share a Print menu on the medication list.

Use Form ES4000 when you need the complete printable profile with diagnosis, physician, and pharmacy details. Use Form ES4010 when you need the weekly Sunday-to-Saturday signing sheet. Read the Report Description before you click View Report.
