How to Print the Letter of Determination (Form H1663) in ElderSuite

Overview

Letter Of Determination - Form H1663 produces the official Form H1663 determination letter for one or more clients. ElderSuite fills client name and mailing address, provider representative name, phone, and mailing address, and today's date, then joins letters into one document in Report Viewer.

Open Report Center, select Nutrition Reports, choose Letter Of Determination - Form H1663. The Report Description reads "Official determination letter for client eligibility in nutrition assistance programs." Opens only from Report Center; changes no client or provider record.

How It Works

1. Open Letter Of Determination From Report Center

Highlight Letter Of Determination - Form H1663 on Nutrition Reports, then click View Report or double-click the name.

2. Select One or More Clients

Under Select Client(s), open the drop-down and check clients by search name.

3. Preview and Print From Report Viewer

Click Preview. If none checked, No Client Selected shows "You must select at least one client before continuing." Preview builds one letter per client dated today, opens Report Viewer, and closes the dialog. Print from the viewer. Cancel closes without producing letters.

Important Limitations

Changes no client or provider record. Letter date is always today. Eligibility outcome boxes (Approved for free meals, Approved for reduced-price meals, Denied, rates, denial reason, fair-hearing contact) print blank for hand completion. Report Center only.

Complete the Determination Boxes by Hand

Preview fills names, addresses, phone, and today's date - not the eligibility decision. Complete outcome boxes before sending or filing.
