How to Print the Claim for Reimbursement Worksheet (Form H4502) in ElderSuite

Preview and print Form H4502, the monthly day-by-day meal-count worksheet, from Report Center Nutrition Reports to support a CACFP or state nutrition claim.

📝 Overview

Claim for Reimbursement Worksheet - Form H4502 is the Report Center report that produces the monthly meal-count worksheet behind a meal reimbursement claim. For the month you choose, ElderSuite fills day-by-day counts of Breakfast, Morning Snack, Lunch, and Afternoon Snack, plus the four monthly totals, onto a blank Form H4502.

Open Report Center from the ElderSuite main menu, select the Nutrition Reports tab, and choose Claim for Reimbursement Worksheet - Form H4502. The Report Description reads "Detailed worksheet for calculating meal reimbursement claims and supporting documentation."

This report opens only from Report Center -> Nutrition Reports. It is not in the Nutrition Center. It sits next to Claim for Reimbursement in the same list - that report builds the monthly claim itself; this one builds the supporting day-by-day worksheet. The finished worksheet opens in Report Viewer. Running it does not submit a claim to a state agency and does not change attendance, client, or contractor records.

âš™ How It Works

1. Open Form H4502 From Report Center

Open Report Center and select Nutrition Reports. In the Report List, highlight Claim for Reimbursement Worksheet - Form H4502.

Click View Report, or double-click the report name. The small Claim for Reimbursement Worksheet - Form H4502 window opens centered on the Report Center. The Report Center cannot be used again until this window closes.

2. Set the Month and Year

The window has two settings only: Select Month and Select Year. There is no reimbursement level, client list, or other filter.

Both fields open on today's month and year, so the current month's worksheet needs no change before you preview. For an earlier month, change Select Month - and step Select Year down when that month falls in a previous year.

One run always covers one whole calendar month, from its first day through its last. Short months and leap-year Februaries are handled correctly. There is no start-date or end-date setting for a day, week, quarter, or year.

3. Preview and Print From Report Viewer

Check the month and year, then click Preview. A Working... indicator appears while ElderSuite loads attendance records for that month, writes the day-by-day meal counts and four monthly totals onto a copy of the blank Form H4502, locks the figures, and opens the finished worksheet in Report Viewer inside the main ElderSuite window. The Form H4502 window then closes.

Preview does not send anything to a printer. Print or save from the viewer. The finished worksheet is locked - figures cannot be typed over. To correct a count, fix the attendance records that carry the meal marks, then reopen the report and preview again.

Click Cancel to close without producing a worksheet. Do not rely on the Esc key in this window.

4. Read What the Worksheet Shows

The heading carries the contractor name and program number from the meal contractor record held for the signed-in session, plus the month and year you chose. The program number is split across two boxes on the form.

Each day of the month has four figures: how many attendance records dated that day were marked for breakfast, morning snack, lunch, and afternoon snack. A day with nothing marked for a meal shows a zero, not a blank. The four total boxes at the bottom are the month's totals for those same four meal services.

Nothing on the worksheet is typed in beyond the month and year. Every figure is counted from attendance records already in ElderSuite.

âś… Important Limitations

This window changes no attendance, client, or contractor record. The only files it writes are temporary worksheet copies on the local computer.

The dialog always closes after Preview, whether a worksheet was produced or not. To run a different month, reopen Claim for Reimbursement Worksheet - Form H4502 from the Nutrition Reports list.

There is no contractor picker on this window. The contractor name and program number come from the meal contractor record for the signed-in session. Enter those details in Meal Contractor Information before you run the worksheet.

Nothing is checked before the worksheet is produced. A future month, a mistyped year, or a month with no meal marks still produces a complete-looking worksheet with a zero in every box and no warning.

Form H4502 is a monthly document only. A longer period must be run one month at a time.

âš  Check the Month Before You Preview

Select Month and Select Year open on today. A supporting worksheet is often needed for a month that has already ended, so change both fields to the period you need, then click Preview.

A page of zeroes usually means the wrong month or year was chosen, or that no attendance records with meal marks exist for that month - not that the report failed. Confirm the period printed in the worksheet heading in Report Viewer before you file or send it.
